This brief self-report measure helps respondents identify health-related behaviors and contextual factors that may contribute to future health decline. The Self-Assessment Questionnaire for Risk Factors for Health Deterioration is intended for rapid screening and to support clinical or educational discussions about modifiable risk factors.
