Emotional State Scale Test - the question form

1. Mood.
-4: Severe despondency, with a bodily sense of it
-3: Despondent mood
-2: Low mood (persistent throughout the illness)
-1: Low mood intermittently (in response to procedures, unpleasant conversations, or parents not showing up)
0: Even mood
+1: Elevated mood (hypomanic; euphoria; silliness)
2. Anxiety, fear.
-4: Marked fear or anxiety
-3: Severe anxiety with motor agitation or psychomotor slowing
-2: Persistent anxiety without outward signs (does not affect behavior)
-1: Anxiety only in response to unpleasant factors (procedures, delayed discharge, parents not showing up)
0: No anxiety
3. Sociability.
−3: Does not answer questions
−2: Does not initiate contact; answers are brief
−1: Selectively sociable with only a few people; speaks little
0: Behavior is appropriate to the situation
+1: Increased drive to socialize; intrusive; interferes in everything
4. Interests and play activity.
-3: Appears completely indifferent to the situation; no play activity.
-2: Interests are restricted (by illness, treatment, procedures, discharge); shows only a formal interest in reading or toys; no creative activity.
-1: Plays, reads, or draws only occasionally, without deep interest.
0: Play activity is not affected by the illness; appropriate for age and situation; active in play, reads a lot, engaged in schoolwork.
+1: Interests are diffusely increased; in games and reading the child is superficial, quickly but unproductively shifts from one activity to another.
5. Attitude toward self.
-3: Extremely exaggerated feelings of worthlessness and guilt
-2: Feelings of guilt and self-deprecation that are realistic in content
-1: Low self-esteem; feelings of guilt without specific content
0: No depressive thoughts
+1: Increased self-esteem; overestimation of one’s condition
6. Motor activity.
-2: Remains in bed all the time
-1: Slow, mildly slowed down
0: Appropriately active (taking physical condition into account)
+1: Increased drive to be active; fidgety, excessively restless
7. Facial expression.
-3: Fixed, mask-like
-2: Exaggerated expression of suffering
-1: During the interview, pale (expressionless)
0: Appropriate to the situation
+1: Inappropriately animated
8. Voice.
−2: Dull, poorly modulated
−1: Quiet, fading
0: Usual voice
+1: Inappropriately loud voice
