Questions: 15 · 10 minutes
1. Which statement best reflects pregnancy, nursing, or recent postpartum changes for you?
I am currently pregnant or nursing.
None applies to me now, and I am not recently postpartum.
I recently gave birth or stopped nursing and may still be experiencing vision or hormonal changes.
I am considering pregnancy in the near future.
2. Which statement best describes your age group?
I am 40 or older.
I am under 18.
I am between 21 and 39.
I am between 18 and 20.
3. Over approximately the past two years, how has your glasses or contact-lens prescription behaved?
It has changed noticeably more than once or is still fluctuating.
It has remained essentially unchanged at my routine examinations.
There was one small change, but it has otherwise been steady.
It changed within the past year, though not repeatedly.
4. Thinking about your corneas and eye surface, which description fits best?
I have recurring dryness, irritation, or inflammation that has not yet been assessed.
I have no known corneal or eye-surface condition and no persistent symptoms.
I have been diagnosed with keratoconus, corneal thinning, or another condition a clinician said could affect laser surgery.
I have mild dryness or irritation that is currently controlled with professional guidance.
5. If a clinician recommended pausing nicotine use around surgery, which response would best match your situation?
I use nicotine occasionally and would want help planning a pause.
I use nicotine daily but believe I could follow a planned pause with support.
I do not currently use nicotine.
I use nicotine regularly and do not expect that I could pause even if the surgeon considered it necessary.
6. During a consultation, the surgeon explains possible glare, halos, dryness, undercorrection, and the chance of needing another procedure. How would you respond?
I would ask how those possibilities apply to my eyes and weigh them against the expected benefits.
I would assume those outcomes happen only when surgery is performed incorrectly.
I would want time to compare their likelihood and impact before deciding.
I would probably proceed but avoid discussing the details because they make me anxious.
7. How thoroughly have your medications and supplements been reviewed for possible effects on laser eye surgery or healing?
A clinician has advised that a current medication or treatment may require postponement or special planning.
I take medications or supplements, but they have not been reviewed specifically for LASIK.
A qualified clinician has reviewed my complete list and identified no current LASIK-related issue.
I do not take regular medications or supplements, but I have not yet completed a formal review.
8. Which statement best describes health conditions that might affect inflammation, infection, or healing?
I have a condition that is generally controlled, but I know it should be discussed before surgery.
A clinician has told me that a current or uncontrolled condition may increase my healing or infection concerns.
I have no known relevant condition and no history of unusual healing problems.
I have a potentially relevant condition that has not been reviewed recently.
9. What outcome are you expecting from LASIK?
I hope to depend less on glasses or contacts, while accepting that results vary and correction may still be needed.
I expect excellent vision immediately and would view a slower recovery as evidence that something went wrong.
I mainly want improvement for particular activities, and I need to clarify what is realistic.
I expect guaranteed perfect vision for life with no possibility of glasses, side effects, or further treatment.
10. You notice how clearly each eye sees with your usual correction. What is closest to your experience?
One eye has long been harder to correct clearly, and this has been evaluated.
Both eyes generally reach clear, stable vision with my current correction.
I have sudden vision loss, severe pain, a curtain-like shadow, or new flashes with many floaters.
One or both eyes do not see clearly even with my current correction, and I have not had this assessed recently.
11. Suppose an advertised discount expires soon, but you still have unanswered questions. What would you most likely do?
Book immediately because the lower price matters more than resolving every concern.
Reserve a date while continuing to gather information, even if cancellation could be inconvenient.
Ask the clinic to clarify the remaining points before committing.
Let the offer go if necessary, although I might first ask whether more decision time is available.
12. After an ordinary cut, dental procedure, or previous operation, what usually happens?
Healing has generally been typical and uncomplicated.
I am currently dealing with an infection, open wound, or significant unresolved healing problem.
I have occasionally healed more slowly than expected, without a major complication.
I have had repeated delayed healing, significant scarring, or postoperative infections that need discussion.
13. If your examination showed that another option was more suitable than LASIK, what would you be inclined to do?
Seek additional opinions until someone agreed to perform LASIK, regardless of the examination findings.
Ask about the reasoning and compare appropriate alternatives before deciding.
Feel disappointed and need time before considering a different approach.
Continue pursuing LASIK unless the concern sounded immediately dangerous.
14. What best describes your history of eye injury, infection, or surgery?
I had a past issue that resolved, and an eye professional knows about it.
I currently have an eye infection or injury, or I am recovering from recent eye surgery.
I have had no significant eye injury, infection, or surgery.
I had a significant past eye event that has not been reviewed in relation to LASIK.
15. How prepared are you to manage transportation, prescribed drops, activity restrictions, and follow-up visits?
I have not considered those arrangements and may not be able to follow the early instructions.
I understand the main requirements but still need to confirm one or two arrangements.
I could manage most instructions, although attending every follow-up may be difficult.
I can arrange transportation, obtain prescribed supplies, follow restrictions, and attend scheduled reviews.
