One simple measure usually provides adequate relief.
I need repeated or combined measures and still have some disruption.
Pain remains difficult to control, or I cannot safely use available measures and the pain is substantial.
2. Outside your period, how often do you experience unexplained aching, pressure, or sharp pain in the lower abdomen or pelvis?
Never.
On isolated occasions.
On several days in a typical month.
Weekly or more often, or in prolonged episodes.
3. If vaginal penetration is part of your experience, which description best fits deep pelvic pain during or afterward?
Penetration is not part of my experience, or I have not had deep pelvic pain with it.
I have occasionally noticed mild, brief discomfort.
Deep pain recurs and leads me to pause, change activities, or manage discomfort afterward.
Even once, deep pain has been intense, prolonged, or accompanied by faintness or significant bleeding.
4. What effect do these symptoms have on your sleep?
They do not disturb my sleep.
They occasionally delay sleep or wake me briefly.

Even once, pain or bleeding has kept me awake for most of the night or made me feel faint when getting up.
5. Which statement best describes bladder discomfort or pain with urination around your period?
I have not experienced it.
I have noticed mild discomfort on an isolated occasion.
It recurs around some periods or when my bladder is full.
It is severe, repeatedly cycle-linked, or makes me avoid urinating despite the need to go.
6. How closely do bloating, diarrhea, constipation, or bowel cramping follow your menstrual cycle?
I have not noticed a cycle-linked pattern.
There may be an occasional mild connection.
One or more symptoms clearly worsen around many periods.
The pattern is strong, recurrent, and substantially disrupts eating, sleep, travel, or routine.
7. Which statement best describes any urgent warning signs accompanying pelvic or abdominal symptoms?
I have not experienced the warning signs listed below.
Pain has worried me, but it was not sudden or severe and did not include fainting, fever, heavy bleeding, or repeated vomiting.
I have had recurrent or escalating episodes that felt urgent, although none included the listed emergency features.
Even once, I have had sudden severe or one-sided pain, fainting, pain with possible pregnancy, fever, uncontrollable vomiting, or bleeding soaking a pad or tampon about hourly.
8. Have you noticed blood that appeared to come from your urine or stool, particularly around a period?
No, I have not noticed this.
I saw a possible trace once, but the source was unclear.
I have noticed repeated traces, although I could not confirm the source.
Even once, I clearly passed blood in urine or stool, or the bleeding was substantial or recurrent.
9. When pelvic pain occurs around a period, how does its timing usually unfold?
I have not noticed pelvic pain linked to my periods.

It often starts before bleeding or lasts through several days of the period.
It starts well before bleeding, continues afterward, or occupies much of the cycle.

Even once, I have been unable to attend or complete an essential activity because of the symptoms.
11. Looking across recent menstrual cycles, how persistent is the overall symptom pattern?
I have not noticed a recurring pattern.
I noticed symptoms during one cycle or an isolated episode.
A similar pattern has appeared across two or three cycles.
The pattern has continued across four or more cycles, is worsening, or occurs between periods as well.
12. When pain, bleeding, or bowel or bladder symptoms flare, how are your energy and ability to concentrate affected?
I do not notice an effect.
I feel somewhat more tired or distracted but can follow my routine.
I repeatedly need extra rest or have difficulty concentrating on normal tasks.
Even once, exhaustion, weakness, or poor concentration has made normal functioning impossible or felt physically alarming.
13. Around your period, what happens when you have a bowel movement?
I do not experience pain or unusual pressure.
I occasionally notice mild discomfort.
Bowel movements are predictably painful or pressured during many periods.
Even once, the pain has been severe enough to avoid bowel movements, feel faint, or be unable to continue normally.
14. During the past six months, what has period pain typically been like at its worst?
I have not had period pain.
